Maltepe Üniversitesi, Lisansüstü Eğitim Enstitüsü, İstanbul.https://hdl.handle.net/20.500.12415/12580AMAÇ: Çalışmamızın amacı günümüzde yaygın olarak kullanılan peruktan kifoplasti uygulamasında genel anestezi ve lokal anestezi yöntemlerinde ağrı değerlerini karşılaştırmaktır. MATERYAL VE METOD: Bu retrospektif çalışma Maltepe Üniversitesi Tıp Fakültesi Hastanesi Ortopedi ve Travmatoloji Ana Bilim Dalı’nda Ocak 2021 ile Kasım 2022 tarihleri arasında gerçekleştirildi. Çalışmaya dahil edilen hastalar arasında basit randomizasyon yöntemi ile 13’ü çalışma (lokal anestezi) 15’i kontrol (genel anestezi) grubu olarak iki grup oluşturulmuştur. Hastaların preoperatif ve postoperatif VAS skorları kaydedilmiş ve karşılaştırılmıştır. Hastaların sosyal yaşam kalitesi bilgileri toplanarak ODS skorları belirlenmiş ve karşılaştırılmıştır. Ayrıca her iki grubun ameliyat süreleri kaydedilerek karşılaştırılmıştır. BULGULAR: İki grup arasında preoperatif VAS değerleri anlamlı (p > 0.05) farklılık göstermemiştir. Çalışma grubunda postoperatif VAS değerleri kontrol grubundan anlamlı (p < 0.05) olarak daha düşük bulunmuştur. Kontrol grubunda postoperatif VAS değerleri preoperatif döneme göre anlamlı (p < 0.05) olarak daha düşüktü. Çalışma grubunda postoperatif VAS değerleri preoperatif döneme göre anlamlı (p < 0.05) olarak daha düşüktü. Çalışma grubunda postoperatif VAS değerlerinin düşüşü kontrol grubundan anlamlı (p < 0.05) olarak daha yüksek olarak bulunmuştur. Kontrol grubunda ameliyat süresi çalışma grubundan anlamlı (p < 0.05) olarak daha yüksekti. SONUÇ: Bu çalışmada lokal ve genel anestezi altında yapılan kifoplasti yönteminde preoperatif ve postoperatif VAS ve ODS değerleri bulunarak karşılaştırılmıştır. Gruplar arasında preoperatif VAS ve ODS değerleri arasında anlamlı fark bulunmamıştır. Preoperatif ve postoperatif VAS ve ODS değerleri karşılaştırıldığında gruplar arasında anlamlı fark bulunmuştur. Çalışma grubu hastalarında anlamlı olarak daha düşük VAS ve ODS değerleri tespit edilmiştir. Çalışma grubunda ameliyat süresinin genel anestezi grubundan %50 oranda daha kısa süreye sahip olduğu bulunmuştur.OBJECTIVE: The primary aim of our study is to compare the pain levels in percutaneous kyphoplasty, a commonly utilized procedure today, between general anesthesia and local anesthesia modalities. MATERIALS AND METHODS: This retrospective study was conducted at Maltepe University Faculty of Medicine Hospital's Department of Orthopedics and Traumatology between January 2021 and November 2022. Using a simple randomization method, among the included patients, 13 were designated as the study group (local anesthesia) and 15 as the control group (general anesthesia). Preoperative and postoperative VAS scores of the patients were recorded and compared. Patients' quality of social life data was collected and their ODS scores were determined and compared. Furthermore, the operation durations for both groups were recorded and compared. RESULTS: No significant difference (p > 0.05) was observed in preoperative VAS values between two groups. Postoperative VAS values in the study group were found to be significantly (p < 0.05) lower than the control group. In the control group, postoperative VAS values were significantly (p < 0.05) reduced compared to the preoperative period, similarly in the study group. The decline in postoperative VAS scores in the study group was significantly (p < 0.05) greater than in the control group. The operation duration in the control group was significantly (p < 0.05) longer than the study group. The median operation time of the study group was found to be 50% less than the median operation time of the control group. CONCLUSION: In this study, preoperative and postoperative VAS and ODS values were obtained and compared for the kyphoplasty method performed under both local and general anesthesia. No significant difference was found between the groups in terms of preoperative VAS and ODS values. However, when comparing preoperative and postoperative VAS and ODS values, a significant difference was identified between the groups. The study group patients exhibited significantly lower VAS and ODS values.
