The patient had noticed a line on her thumbnail 10 days before admission, and also reported a sensation of something similar to nail fungus under her fingernail. She had no other dermatological disease and no history of trauma. Dermatological and dermoscopic examination revealed irregularity of the distal border of the lunula, punctuate and longitudinal leuconychia located on a slightly erythematous background, and distal onycholysis (Fig. 1a,b). A potassium hydroxide preparation of the nail plate was negative for fungal elements. An excisional biopsy was taken from the lesion, representing papillary projections on the nail bed (Fig. 2a–d).
