Yayın An Examination of the Propaganda Posters in the 1980 Nicaragua Literacy Campaign in the Context of Reading Books(Turkish Librarians Assoc, 2021) Gazi, Mehmet Ali; Caki, Caner; Karaca, Mustafa; Caki, GülThe Sandinista National Liberation Front (Frente Sandinista de Liberacion Nacional, FSLN) took over the administration of the country as a result of the revolution that took place in Nicaragua in 1979 and started a literacy campaign in 1979. The fact that FSLN's founder, Carlos Fonseca, is a librarian and a teacher, made the campaign gain a special importance. Propaganda posters were used effectively to emphasize the importance of literacy and reading books in the campaign. The campaign was awarded the UNESCO Nadezhda K. Krupskaya literacy award in 1980. It was tried to reveal how propaganda posters were used in literacy campaign in order to encourage literacy and reading books in the study. For this purpose, propaganda posters were accessed from the International Institute of Social History (Internationaal Instituut voor Sociale Geschiedenis) in Amsterdam and visual and written indicators in propaganda posters were analyzed in the light of three dimensions in semiotics of the US linguist Charles William Morris. In the findings obtained, it was revealed that efforts were made to encourage people to be literate and to read books asserting the perception that literacy liberates peopleand identifying literacy with revolution through the propaganda posters.Yayın Giant craniofacial immature teratoma with primary intracranial lesions(Wolters Kluwer Health, 2010) Karaca, Mustafa; Akan, Mithat; Silav, Gökalp; Aköz, TayfunTeratomas can occur in almost any region of the body and are the most common extragonadal germ cell childhood tumors. Immature teratomas in the head and neck region cause severe deformity and threat life because of several reasons. Imaging investigation (ultrasonography, computed tomography, magnetic resonance imaging) is of outmost importance for the diagnosis and preoperative planning, considering that the total surgical resection of the teratoma constitutes the treatment of choice. Untreated teratomas have mortality rates of 80% to 100% . In this study, a giant craniofacial immature teratoma having both intracranial and extracranial primary lesions is reported. As a conclusion, intracranial primary lesions should be kept in mind in extracranial head and neck immature teratoma cases.Yayın Is polymethylmethacrylate reliable and practical in full-thickness cranial defect reconstructions(Wolters Kluwer Health, 2011) Akan, Mithat; Karaca, Mustafa; Eker, Güniz; Karanfil, Hüseyin; Aköz, TayfunObjective: The study aimed to evaluate the success of polymethylmethacrylate cranioplasty combined with various soft tissue coverage techniques in repairing full-thickness cranial defects, to compare our results with similar studies published before, and to describe a simple method of implant premolding. Methods: A total of 17 patients who had cranial defects due to various etiologies underwent polymethylmethacrylate cranioplasty. In 10 patients, the implant premolding method was applied. The soft tissue coverage was obtained by primary closure, local flaps, or free flaps. Results: The follow-up period ranged from 36 hours to 5 years. Only 1 implant became exposed among the 17 patients. One patient died 36 hours after the surgery because of myocardial infarction. The remaining 15 patients had no early or late postoperative complications. Good contour restoration and stable reconstruction of the calvarial defects were realized, and a successful combination of various soft tissue coverage techniques was achieved. Conclusions: We concluded that polymethylmethacrylate was a cheap and durable material, useful in full-thickness calvarial defect reconstructions. It can be combined with any soft tissue coverage techniques except skin grafting. In most cases, the simple premolding methods are useful for both defect matching and preventing tissue damage due to exothermic reaction of the material.Yayın Lower lip reconstruction with neck flaps as a salvage procedure(Wolters Kluwer Health, 2010) Yıldırım, Serkan; Karaca, Mustafa; Bilgiç, Mehmet İlker; Aköz, TayfunThere are many surgical reconstructive techniques after lower lip ablation. As the lower lip defect gets larger, the required reconstructive techniques become more complex. Despite the indication of free flaps, some situations require local flaps for reconstruction. These situations may be donor and/or recipient vessel problems or systemic disorders of the patient. In this article, a total lower lip reconstruction technique using the neck skin was described. The technique was applied successfully in 2 cases. An acceptable cosmetic result was obtained, and there were no early or late surgical complications in 2 years’ follow-up. The patients were satisfied with the results. Lower lip reconstruction with the neck flaps must be kept in mind as a quicker alternative and a salvage method when the free flaps cannot be applied.Yayın Marjolin ulcer of the scalp: intruder of a burn scar(Wolters Kluwer Health, 2008) Taylan, Gaye; Akan, Mithat; Karaca, Mustafa; Aköz, TayfunChildhood burn accidents are still a problem all over the world. Besides the contractures and hypertrophic scar conditions, malignant transformation in the burn scar is one of the primary concerns later in adulthood. Marjolin ulcer, commonly seen after burn scar, is a neoplastic change in the scar tissue. The scalp region necessitates additional attention because of the inevitable structures it protects. The long-standing ulcers with malignant transformation may cause invasion of different layers of the scalp. As the cranium is invaded, reconstruction after wide excision of these tumors becomes more difficult to deal with. Scalp invasion of Marjolin ulcers with different levels is presented in the following study. Consistent with the literature, histopathology of the tumors was squamous cell carcinoma in most patients. Although rare, mesenchymal tumor is involved in 2 of 9 patients. The latent period of the tumor is inversely proportional to the age at the time of burn injury. As the patient is younger at the time of injury, the occurrence of the ulcer is longer than expected. Lag period as long as 81 years is detected in the study. The delayed diagnosis due to social considerations such as financial limitations increases the likelihood of cranial invasion. A single huge scalp flap is often sufficient for soft tissue defects, and cranioplasty with methyl methacrylate is an appropriate option for reconstruction.Yayın A method for flap monitoring based on bioelectrical-resistance analysis(Wolters Kluwer Health, 2011) Mısırlıoğlu, Aykut; Karaca, Mustafa; Avcı, Gülden; Aköz, TayfunAxial pattern flaps are characterized by a single vascular pedicle that supplies the whole flap and carry the danger of flap loss. For this reason, monitoring and early identification of vascular perfusion is important for flap prognosis. The most commonly used method for flap monitoring is clinical assessment, but it has certain limitations. The bioelectrical properties of tissues can be affected by ischemic conditions, and a preliminary study showed that the bioelectrical resistance of blood was decreased under thrombotic conditions. In this controlled study, the resistance values of the axial flaps were calculated during thrombosis. A total of 28 rats were randomly divided into a control group (n = 14) and an experimental group (n = 14). Axial flaps were elevated based on the inferior epigastric pedicle. For the ischemic group, microvascular clamps were placed in the ischemic group to produce artificial thrombosis from day 1 of the experiment. Bioelectrical resistance was measured every 30 minutes for the first 6 hours and then daily after that. The resistance values from the ischemic group decreased progressively, and 150 minutes after the clamping, the difference became statistically significant (P G 0.05). The results showed that the assessment of flap resistance is a valuable tool and may allow early recognition of a compromise in the vascular system before clinical signs become obvious.Yayın Primary cutis verticis gyrata and scalp reduction in one stage with multiple pinwheel flaps (Revisited)(Wolters Kluwer Health, 2008) Mısırlıoğlu, Aykut; Karaca, Mustafa; Aköz, TayfunCutis verticis gyrata (CVG) is an unusual condition that typically occurs on the scalp and is characterized by the formation of ridges and furrows. These changes resemble the outer surface of the cerebrum. In this study a case of primary CVG is presented, without association of neurologic or neuropsychiatric diseases, which was successfully treated by using one-stage scalp reduction procedure with the pinwheel flap technique. A 46-year-old woman had first noticed the formation of ridges over the vertex of the scalp 20 years earlier. During rhinoplasty, trauma to the septum with elongation of a fracture up to the cribriform plate may result in an osteomeningeal breach in the floor of the anterior cranial fossa. This complication can be treated conservatively, but surgical intervention is necessary in the case of persistent cerebrospinal fluid leakage.1 A 45-year-old woman was referred to as having problems of respiration by nasal airway and posterior nasal leakage. Two nasal surgeries were noted for her medical history. In her examination, there was a loss of inner osteocartilaginous framework of the nose and perforation in the anterior nasal septum. Bu nadir görülen tümörün spesifik bir makroskobik görünümü olmadığından, sık görülen diğer tümöral lezyonlarla karıştırılabilir. Çalışmamızda üst dudağında kondroid siringoma olan otuzyedi yaşında erkek bir olgu sunularak bu nadir tümörün tanı ve tedavisi hakkında literatürdeki bilgiler gözden geçirilmiştir.
