Yayın The evolution of radiological lesions due to septic pulmonary embolism [Septik pulmoner emboliye bagli{dotless} radyolojik bulgulari{dotless}n evrimi](2011) Levent E.; Sariman N.; Soylu A.C.A 22 year-old female patient presented with septic pulmonary embolism (SPE) due to infective arthritis with demonstrative thorax CT images that provided early diagnosis and showed the evolution of radiological lesions. Thoracic CT revealed multiple nodular lesions and areas of consolidation with various sizes on both lung fields. Some of them were cavitating and some were pleurally based. Methicillin sensitive Staphylococcus aureus was obtained on culture as the etiologic agent. Compared to surgical therapies, these methods are comfortable and safe for the patients. Methods: We used therapeutic bronchoscopic methods (laser and stenotic silicon stent) in seven patients who were admitted to our department due to tracheal stenosis, which developed after tracheostomy and endotracheal intubation. In 4 patients after vaporization of membranous stricture by YAP-laser, stenotic stent was implanted mechanically and/or by means of balloon dilatation. The membranous stricture area was coagulated by YAP-laser in 3 other cases and anatomic airway diameter was maintained mechanically and by balloon dilatation. Results and Conclusion: In the follow up period, we applied a stenotic silicone stent after second laser resection to patients, who developed restenosis. Patients who had stenotic silicon stenting had no problem related with the stent in the follow up period were considered as cured.Yayın PAP therapy for OSA(Edizioni Minerva Medica, 2014) Sariman N.Obstructive sleep apnea (OSA) is a commonly encountered chronic disease characterized by repeated airway collapse during sleep leading to nocturnal hypoxemia, sleep fragmentation and daytime sleepiness. Patients usually have important comorbid disorders such as hypertension, coronary artery disease, and diabetes mellitus. Snoring, excessive daytime sleepiness, and witnessed apneas are the most frequent complaints that bring the patient to a sleep specialist. The gold standard test for the diagnosis is polysomnography. OSA is a treatable disorder. Positive airway pressure (PAP) therapy is a very effective non-invasive mechanical treatment method for patients with moderate-to-severe OSA when applied regularly. Objective data achieved during diagnostic polysomnography and titration study assist in the determination of pressure, ventilation mode of the device, and oxygen need for the particular patient. Effectiveness of PAP therapy continues as long as the treatment is being used. PAP therapy prevents both short and long term complications of sleep disordered breathing. Untreated OSA is associated with various adverse health consequences such as stroke, heart failure, cardiovascular disease, and motor vehicle accidents. Compliance during PAP therapy is still an unresolved question and is a limiting factor in treating OSA patients. Since the condition is life-long, it is important to ensure long term use. PAP therapy neccessitates close follow-up, and a multilayered approach using technological and behavioral interventions.Yayın Short-term positive airway pressure therapy response in obstructive sleep apnea patients: Impact of treatment on the quality of life [Obstrüktif uyku apne olgularinda pozitif hava yolu basinci tedavisine erken yanitin degerlendirilmesi: Tedavinin yaşam kalitesi üzerine etkisi](Ankara University, 2012) Yurtlu Ş.; Sariman N.; Levent E.; Soylu A.C.; Alparslan S.; Saygi A.Introduction: We aimed to assess the compliance of obstructive sleep apnea (OSA) patients of whom we planned positive airway pressure (PAP) therapy by using "Calgary Sleep Apnea Quality of Life Index (SAQLI)","Epworth Sleepiness Score (ESS)","OSAS Symptoms Questionnaire (OSQ)" and, to investigate the early effects of treatment on the quality of life. Patients and Methods: A total of 30 adult (male/female: 23/7) OSA patients who applied to Sleep Research Laboratory at the University Hospital, complaining of symptoms related to sleep and polysomnographically verified as OSAS with PAP therapy indications were included to the study. Their written consent were obtained. Results: Characteristics of the patients, OSAS symptoms, ESS and SAQLI sores were recorded. After a month, on the second visit, ESS, SAQLI and OSAS symptoms questionnaire had been repeated. All the patients have routinely used PAP devices for a period of a month. PAP therapy provided significant improvements in excessive daytime sleepiness, symptoms questionnaire and SAQLI scores (p< 0.001). There was a significant correlation between apnea hypopnea index (AHI) score and the improvements in emotional functioning (r = -0.374, p= 0.045). Conclusion: We concluded that the OSA patients can have remarkable benefits from PAP therapy during the early treatment period. This study increased the awareness of the patients about their illness and their perceived benefits related to PAP treatment. It could also be another simultaneous primary lung malignancy. Determining the etiology of the new lesion is important in order to decide the therapeutic management and prognosis. Simultaneous renal and lung cancer is a rare condition and can usually be detected randomly during scanning for metastases. Compared by surgical therapies it is comfortable and has no risks for the patients. For five patients who applied our clinic after tracheostomy and endotracheal intubation stenosis we used therapeutic bronchoscopic methods; "laser-stenotic silicon stent". In two patients after vaporization of membranous stricture by Neodimum Yttrium Aliminum Pevroskite Laser (Nd-YAP laser) who were seen posttracheostomy and postentubation; stenotic stent was implanted mechanically and/or by means of baloon dilatation. Membranous stricture area was coagulated by Nd-YAP-laser in other three cases and anatomic airway diameter was achieved mechanically and by baloon dilatation. In the follow up period we applied stenotic silicon stent implantation after second laser resection in whom restenosis observed. In conclusion; patients who had stenotic silicon stent implantation and having no problems in the follow up this therapeutic method is found to be curative.
