Introduction: The Republic of North Macedonia (North Macedonia) is the fourth largest producer of raw tobacco leaf in Europe, representing over 11% of Europe’s tobacco production. Smoking prevalence in North Macedonia is among the top ranking - 46% of adults aged 15-64 and 12.4% of children aged 13-15. Aim of this study was to explore the attitude of medical students towards smoking cigarettes. Materials and Methods: A cross-sectional observational on line anonymous study was implemented among medical students in N. Macedonia. Questionnaire was electronically distributed using Google Forms platform through the Macedonian Medical association of students. Students were invited several times to fulfill the questionnaire in a period of 6 months. The analysis was performed using RStudio. Results: Total of 260 students aged 18-39 years, who smoke 12 cigarettes per day; 4.8 years in average have answered the questionnaire. Out of them, 173 (66.5%) were female and 87 (33.5%) were male. For every one year increase in age, the odds of thinking that smoking makes you look more serious decrease by 18.57% (OR, 0.8174; 95% CI, 0.7068 - 0.9510, p < 0.01), the odds of thinking that smoking is modern decrease by 14.38% (OR, 0.8562; 95% CI, 0.7262 - 0.9887, p = 0.049) and the odds of thinking that smoking relaxes you decrease by 11.12% (OR, 0.8888; 95% CI, 0.8217 - 0.9531, p < 0.01). Friends who are smoking didn’t statistically influent on smoking cigarettes. Discussion and Conclusions: For female students at Faculty of Medicine, smoking cigarettes is important for more serious and modern look and way of relaxation. It is very important to work on increase of self-confidence and introduce techniques for relaxation other that smoking cigarettes in the first years of Medical studies.
