Post COVID-19 syndrome: reaction of skin and its appendages

Introduction: For a year now the whole world has been living in the COVID-19 pandemic and the morbidity is growing steadily. According to the updated “Coronavirus disease 2019 treatment guidelines”, patients with ? moderate and severe course of this illness require an antibacterial therapy combined with systemic corticosteroid. Due to significant impact of the disease itself on the patients` immunity and hemostasis it looks that well-known side effects of mentioned above drugs tend to produce long-term results in people, who have recovered from COVID. Materials and methods: We have done a retrospective analysis of 87 patients’ medical documentations. 57 females and 30 males, aged 28-63 years, applied for dermatological help on the 10-63thday after the coronavirus disease symptom onset, which was confirmed by a PT-PCR test. All of them suffered from a moderate to severe form of COVID-19: 59 patients received outpatient treatment, 28 – inpatient treatment, including 10 of them, who were admitted to intensive care unit and required supplemental oxygenation. Treatments included injectable antibiotics (cephalosporins, macrolides, phtorquinolons) and dexamethasone within 5-16 days. Results: Detailed dermatological examination detected the following problems developed in research group patients after COVID-19 episode: Telogen e uvium - in 45 patients, T.versicolor - in 39, onychodystrophy – 34, candidiasis of big folds or oral mucosa – 16, urogenital candidiasis - 15, flat warts - 7, common warts – 6, lichen planus – 2. In 58 patients 2 or even 3 nosology at the same time were diagnosed. All these pathological processes manifested within 1-3 months after the disease, some of them – while taking medicines for COVID-19. Discussion and conclusions: The analysis of dermatological morbidity shows that diagnosed skin problems appeared due to side effects of medicines, which were used for the treatment of COVID-19 and cannot be a direct complication of the coronavirus disease itself. A massive antibacterial therapy with systemic corticosteroids leads to immunosuppression, hormonal disbalance. In combination with COVID-19 impact on the patient’s immunity and psychological stress it provokes the activation of fungi and papillomavirus through suppression of beneficial bacteria. Reactive hair loss, onychodystrophy, and lichen planus may be in addition the result of a toxic influence of antibiotics or SARS-CoV-2 on the skin and its appendages.
