Coronavirüs hastalığı 19 tanılı hastaların tedavi sonrası duygu durumlarının değerlendirilmesi

Özet
Amaç: Bu çalışmada Covid-19 sebebi ile hastanede yatarak tedavi gören ve şifa ile taburcu olan hastaların duygu durumlarının ve post-travmatik stres bozukluğunun covid-19 ile ilişkisinin araştırılması amaçlanmıştır. Materyal-Metotlar: Çalışmaya 15 Mart 2020-30 Ekim 2020 tarihleri arasında Akdeniz Üniversitesi Tıp Fakültesi Hastanesine Covid-19 tanısı ile yatıp şifa ile taburcu olan hastalar alındı. Hastalara taburcu olduktan en az 30 gün sonra telefon ile ulaşılıp, postravmatik stres bozukluğu (PTSB) soru listesi-sivil versiyonu (PCL-5), Beck anksi- yete ve depresyon ölçekleri uygulandı. Telefon ile cevap vermek istemeyen ancak taburculuk sonrası poliklinik kontrolüne gelen hastalarda ise bu anketler yüz yüze uygulama şeklinde yapıldı. Anket uygulamaları araştırmacılar tarafından yapıldı. Çalışmaya oral ya da nazal sürüntü sonucu covid PCR pozitif olan hastalar dahil edildi. Bulgular: Akdeniz Üniversitesi Hastanesi Pandemi Kliniğine 15 Mart 2020-30 Ekim 2020 tarihleri arasında 385 hastanın yatarak tedavi gördüğü tespit edildi. Bu hastalardan %55,8 (215) ine telefon ile ulaşıldı. Çalışmaya ortalama yaşı 54,8±14,5 olan 131 (%60,9) erkek, 84 (%29,1) kadın hasta dahil edildi. Çalışmaya dahil edilen hastalar taburcu olduktan ortalama 87,9±43,4 gün sonra çalışmaya alınmıştır. Hastaların ilk tanı anında en sık şikayetleri, ateş (%26,5), dispne (%18,6) ve öksürük (%13,5) iken, çalışmaya alındığı dönem hastaların %38,1 (82)’inde rezidü semptomlar vardı. Hastaların hastanede yatış süresi ortalama 10,8 gün iken, yoğun bakım ihtiyacı olanların yoğun bakımda ortalama yatış süresi 9,7 gün olarak saptandı. Beck depresyon ve anksiyete skorları için cutt off değer olarak 16 puan alındığında sırasıyla depresyon görülme oranı %6,6, anksiyete görülme oranı %13,1 tespit edilmiştir. PCL-5 için 39 cutoff olarak alındığında PTSB %7 oranında saptanmıştır. Çalışmaya alındığı dönem (taburculuktan ortalama 87,9 gün sonra) rezidü şikayeti olan hastaların hem depresyon skoru (p<0,001), hem anksiyete skoru (p<0,001) hem de PCL-5 (p=0,001) skoru istatistiksel olarak anlamlı düzeyde yüksek tespit edildi. Lojistik Regresyon Analizi sonuçlarına bakıldığında rezidü semptom, bekar veya dul/ayrılmış olmak değişkenlerinin anlamlı farklılık yarattığı bulunmuştur. Rezidü semptomu olanların rezidü semptomu olmayanlara göre 6,7 kat daha fazla anksiyete puanına sahip olduğu söylenebilir. Ayrıca, bekar olanlar ile dul/ayrılmış olanlar evli olanlara göre sırasıyla 4,4 ve 17,9 kat daha fazla anksiyete puanına sahiptir. Sonuç: Sonuç olarak covid-19 nedeniyle tedavi görmüş hastaların azımsanmayacak bir bölümü mental hastalıklardan muzdariptir. Özelikle rezidü semptomları olan, tek yaşayan ve kortikosteroid tedavisi gören hastaların bu açıdan daha dikkatli değerlendirilmesini ve gerekirse taburculuk sonrası psikiyatrik destek alması gerektiğini düşünüyoruz.
Aim: The present study aimed to examine mood disorders in patients discharged from the hospital due to Coronavirus Disease-19(COVID-19). Material-Methods: The study included patients who were admitted to Akdeniz University Medical Faculty Hospital with the diagnosis of COVID-19 and discharged between March 15, 2020 and October 30, 2020. Patients were contacted by phone at least 30 days after discharge, and posttraumatic stress disorder (PTSD) questionnaire-civil version (PCL-5), Beck anxiety and depression scales were administered. In patients who did not want to answer by phone but came to the outpatient clinic after discharge, these questionnaires were administered face-to-face. Survey applications were made by the researchers. Patients with positive covid PCR as a result of oral or nasal swab were included in the study. Results: There were 385 individuals treated as inpatients in the Akdeniz University Hospital COVID-19 Referral Clinic between March 15, 2020 and October, 30 2020. We were able to contact 215 (55.8%) of these patients. The median age was 56 years among the patients reached and 60.9% (131) were male. The study patients were included in the study after a mean duration common complaints of the patients reported at COVID-19 diagnosis were fever (26.5%), dyspnea (18.6%), and cough (13.5%), while 38.1% (82) of the patients had residual symptoms at the time of enrollment. The length of hospital stay of the patients was 8 (1 90) days, and the mean length of stay in the intensive care unit for those requiring intensive care was 9.7 days. With a cutoff point of 16 for Beck depression and anxiety scores, the incidence rates of depression and anxiety were 6.6% and 13.1%, respectively. With a cutoff point of 39 for PCL-5, the rate of PTSD was 7%. The depression scores (p<0.001), anxiety scores (p<0.001) and PCL-5 (p=0.001) scores were statistically significantly higher in patients with complaints at the time of enrollment (after a mean duration of 87.9 days following discharge). The results of the Logistic Regression Analysis on Anxiety revealed that residual symptoms and being single or widowed/divorced caused a significant difference. Accordingly, it is possible to say that those with residual symptoms had 6.7 times higher anxiety scores than those without residual symptoms. In addition, those who were single and those who were widowed/divorced had 4.4- and 17.9-times higher anxiety scores, respectively than those who were married. Conclusion: As a result, a substantial portion of patients treated for covid-19 suffer from mental illnesses. We think that especially patients with residual symptoms, living alone and receiving corticosteroid therapy should be evaluated more carefully in this regard and should receive psychiatric support after discharge if necessary.
