dc.description.abstractParoksismal nokturnal hemoglobinüri (PNH), periferik kanda sitopeni, tromboz ve hemolitik anemi ile ortaya çıkan nadir bir kemik iliği yetersizlik hastalığıdır. CD55 ve CD59 isimli iki glikozilfosfotidilinositol (GPI) çapası olan proteinlerin yokluğu nedeniyle kontrolsüz kompleman aktivasyonuna bağlı hemoliz ve diğer PNH bulguları ortaya çıkar. Erken tanı bu hastaların prognozu ve yönetiminde temeldir. Eculizumab, kronik kontrolsüz kompleman aktivasyonunu spesifik olarak inhibe eden ve terminal kompleman yolundaki C5’i hedef alan monoklonal antikordur. Çocuklarda ve erişkinlerde PNH tedavisi için onaylı ilk ve tek ilaçtır. Bu yazıda demir eksikliği anemisi tablosu ile başvuran ve Eculizumab ile başarılı şekilde tedavi edilmiş bir PNH olgu sunulmaktadır.en_US
dc.description.abstractParoxysmal nocturnal hemoglobinuria (PNH) is a rare bone marrow failure disorder that manifests with hemolytic anemia,thrombosis,and peripheral cytopenias. The absence of two glycosylphosphatidylinositol (GPI)-anchored proteins, CD55 and CD59,- leads to uncontrolled complement activation that accounts for hemolysis and other PNH manifestatitions. Hence, early diagnosis is essential for improved patient management and prognosis.Eculizumab specifically inhibits chronic, uncontrolled complement activation and is a monoclonal antibody targeting C5 within terminal complement pathway. Eculizumab is the first and only approved treatment for PNH adults and children. We herein presenting a case presented with iron deficiency anemia, diagnosed as PNH and treated with Eculizumab.en_US
