Özet
Bu çalışma sağlık çalışanlarında kalite algısını iki devlet hastanesinin karşılaştırması ile ortaya koymak amacıyla yapılmıştır. Bu araştırma, verilerin anket yöntemi kullanılarak temin edildiği tanımlayıcı bir araştırmadır. Araştırmanın evrenini, İstanbul ilinde bulunan Sultanbeyli Devlet Hastanesi ve Tuzla Devlet Hastanesi (iki kamu hastanesi) çalışanları oluşturmaktadır. Her iki hastanede de örnekleme yöntemi olarak basit tesadüfi örnekleme yöntemi kullanılmıştır. Araştırma kapsamındaki Sultanbeyli Devlet Hastanesinde araştırmaya katılan kişi sayısı 161, Tuzla Devlet Hastanesinde araştırmaya katılan kişi sayısı 173’tür. Toplam katılımcı sayısı 334’tür. Araştırma verilerinin toplanmasında, Bayer ve Baykal (2017) tarafından geliştirilmiş olan “Sağlık Çalışanlarında Kalite Algısı Ölçeği” olmak üzere toplam iki bölümden oluşan anket formu kullanılmıştır. Katılımcıların %48,2’sinin Sultanbeyli, %51,8’inin Tuzla devlet hastanesinde görev yaptığı, %36’sının erkek, %74’ünün kadın olduğu ve yaş ortalamalarının 33,87±8,7 olduğu görülmüştür. Çalışmaya en fazla katılım önlisans/lisans mezunlarından olmuştur. Katılımcıların %62,6’sının evli, %37,4’ünün bekar olduğu görülmüştür. Tuzla Devlet Hastanesinde çalışanların kalite eğitimi alma oranının (%80,9) Sultanbeyli Devlet Hastanesinde çalışanların kalite eğitimi alma oranından (%70,8) daha yüksek olduğu görülmektedir. Sağlık çalışanları kalite algısı ölçeği toplam puanında (t=-2,05; p<0,05) katılımcıların verdikleri cevapların ortalamaları Sultanbeyli ve Tuzla hastanelerinde çalışmalarına göre farklılaşmaktadır. Tuzla hastanesindeki sağlık çalışanlarının hastanelerine ilişkin kalite algıları Sultanbeyli hastanesindeki sağlık çalışanlarına göre daha olumludur. Kalite eğitimi alan çalışanların hastanenin hizmet kapasitesini yeterli bulma düzeylerinin (%51,2) kalite eğitimi almayan çalışanların hastanenin hizmet kapasitesini yeterli bulma düzeylerine (%35,0) göre daha yüksek olduğu saptanmıştır. Araştırma sonucunda sağlık personelinin hastaneye yönelik kalite algısında yaş, eğitim durumu, meslek, meslekte çalışma süresi ve çocuk durumu etkili olarak tespit edilmiştir. Elde edilen sonuçlar kapsamında sağlık kurumlarında kalite kültürünün oluşturulması için görev yapan çalışanların kalitenin geliştirilmesi ve uygulanması noktasında aktif bir katılımı sağlanması gerektiği önerilmiştir.
This study was conducted to reveal the quality perception of healthcare professionals by comparing two public hospitals. This is a descriptive study in which data were obtained using the questionnaire method. The population of the research consists of the employees of Sultanbeyli State Hospital and Tuzla State Hospital (two public hospitals) in Istanbul. Simple random sampling method was used as the sampling method in both hospitals. The number of people participating in the research at Sultanbeyli State Hospital within the scope of the research is 161, and the number of people participating in the research at Tuzla State Hospital is 173. The total number of participants is 334. In the collection of research data, a questionnaire consisting of two parts, the "Quality Perception Scale in Healthcare Professionals" developed by Bayer and Baykal (2017), was used. The data obtained were analyzed in the SPSS program, Percent (5), Frequency (N), Mean and standard deviations were calculated, Chi-square analyzes were made, the differences between demographic variables were analyzed with the T test and Anova test. It was observed that 48.2% of the participants worked in Sultanbeyli, 51.8% in Tuzla state hospital, 36% were male, 74% were female, and their mean age was 33.87±8.7. The highest participation in the study was from associate/undergraduate graduates. It was observed that 62.6% of the participants were married and 37.4% were single. It is seen that the rate of receiving quality training (80.9%) of the employees in Tuzla State Hospital is higher than the rate of receiving quality training (70.8%) of those working in Sultanbeyli State Hospital. The average of the answers given by the participants in the total score of the health workers perception of quality scale (t=-2.05; p<0.05) differs according to their work in Sultanbeyli and Tuzla hospitals. The quality perceptions of healthcare professionals in Tuzla hospital regarding their hospitals are more positive than those of healthcare professionals in Sultanbeyli hospital. It was determined that the level of finding the service capacity of the hospital sufficient (51.2%) of the employees who received quality training was higher than the level of finding the service capacity of the hospital sufficient (35.0%) of the employees who did not receive quality training. As a result of the research, age, education level, occupation, working time in the profession and child status were determined to be effective in the quality perception of the health personnel towards the hospital. Within the scope of the results obtained, it has been suggested that the active participation of the employees working in the establishment of a quality culture in health institutions should be ensured at the point of development and implementation of quality.
