dc.description.abstractA 60-year-old woman presented with a 6-month history of painless nail discolouration of the left thumb. The patient had noticed a line on her thumbnail 10 days before admission, and also reported a sensation of something similar to nail fungus under her fingernail. She had no other dermatological disease and no history of trauma. Dermatological and dermoscopic examination revealed irregularity of the distal border of the lunula, punctuate and longitudinal leuconychia located on a slightly erythematous background, and distal onycholysis (Fig. 1a,b). A potassium hydroxide preparation of the nail plate was negative for fungal elements. An excisional biopsy was taken from the lesion, representing papillary projections on the nail bed (Fig. 2a–d). At the 6-month follow-up examination, the leuconychia had almost completely disappeared, the irregularity of the distal border of the lunula had improved, and only distal yellowish discolouration and onycholysis were present (Fig. 1c,d).en_US
