Özet
Bu çalışma, pandemi öncesi ve pandemi döneminde meme sağlığı tarama merkezlerine başvuran hasta sayılarının ve bu hastaların başta sosyodemografik olmak üzere bazı özelliklerinin karşılaştırılması amacıyla yapılmıştır. Araştırma retrospektif olarak planlanmıştır. Meme tarama programı uygulayan Maltepe Üniversitesi Tıp Fakültesi Hastanesi Radyoloji Anabilim Dalı ve özel bir meme tarama kliniği Mart 2019 - Mart 2020 ( pandemi öncesi ) Mart 2020 - Mart 2021 ( pandemi sürecinde ) araştırmanın yapıldığı hastanelere başvuran hastaların başvuru amacı, başvuru sonrası süreçte tanı, tetkik ve tedavi süreçlerinin gelişimleri değerlendirilmiştir. Toplam 7459 farklı hasta kaydı incelenmiştir. Pandemi öncesi yılda Üniversite Hastanesine 206, özel meme tarama kliniğine 1469 hasta meme şikayet veya kontrolü için başvururken pandemi döneminde bu sayılar sırasıyla 127 ve 563’e düşmüştür. Pandemi döneminde pandemi öncesine göre başvurularda sırasıyla % 38 ve % 62 azalma görülmektedir. Meme USG ve mamografi sonuçlarına göre BİRADS kategorileri değerlendirildiğinde Üniversite Hastanesine başvuran hastalarda BİRADS 0 ve BİRADS 5 kategorilerinde, özel meme tarama kliniğine başvuran hastalarda BİRADS 0 ve BİRADS 6 kategorilerinde pandemi öncesine göre istatistiksel olarak anlamlı düzeyde yükseklikler belirlenmiştir. Araştırma sonuçlarına göre pandemi döneminde her iki klinikte de meme hastalarının başvuru sayılarında ciddi azalmalar tespit edilmiştir. Bunun yanısıra hastaların ileri tetkikleri yapıldığında pandemi öncesine göre daha ileri evre bulgulara rastlanmıştır. Bu tür pandemi dönemlerinde hastaların takip ve tedavilerini kendi insiyatiflerine bırakmadan, gerekirse hizmeti hastaların ayağına götürecek çözümler üretmek gerekir. Aksi haled daha fazla sayıda ya da daha ileri evrede meme kanseri hastaları ile uğraşmak zorunda kalınacaktır.
This study was conducted to compare the number of patients who applied to breast health screening centers before and during the pandemic, and some of the sociodemographic characteristics of these patients. These research was planned retrospectively. Maltepe University Medical Faculty Hospital Department of Radiology and a private breast screening clinic that implements a breast screening program March 2019 - March 2020 (pre-pandemic) March 2020 - March 2021 (during the pandemic) The purpose of application of the patients who applied to the hospitals where the research was carried out, diagnosis, examination in the post-application process and the development of treatment processes will be evaluated. A total of 7459 different patient records were examined. In the year before the pandemic, 206 patients applied to the University Hospital and 1469 patients patients to the private breast screening clinic for breast compliants or control, while these numbers decreased to 127 and 563, respectively, during the pandemic period. During the pandemic period, there is a 38% and 62% decreased in applications, respectively compared to the pre-pandemic period. When BIRADS categories were evaluated according to breast USG and mammographyresults, statistically significant elevations were found in BIRADS 0 and BIRADS 5 categories in patients admitted to the University Hospital, and in BIRADS 0 and BIRADS 6 categories in patients who applied to a private breast screening clinic, compared to the pre-pandemic period. According to the results of the research, serious decerases weer detected in the number of applications of breast patients in both clinics during the pandemic period. In addition when the patients were examined further, more advanced findings were found compared to the pre-pandemic period. In such pandemic period, it is necessary to produce solutions that will bring the service to the patients’ feet, if necessary, without leaving the follow-up and treatment of the patients to their own initiative. Otherwise, it will have to deal with more or more advanced breast cancer patients.
