Özet
Amaç: Ocak 1999-Nisan 2003 arasında kliniğimizde yapılan göz çevresi yeniden yapılandırma nedenleri ve sonuçlarının geçmişe dönük incelenmesi ile mevcut yeniden yapılandırma alternatiflerinin avantaj ve dezavantajlarının ortaya konması ve yeniden yapılandırma sırasında dikkat edilmesi gereken noktaların belirlenmesidir.Gereç ve Yöntemler: Elli üç aylık sürede 84 hastaya göz çevresi yeniden yapılandırma uygulanmıştır. Bu hastaların 62sinde defekt nedeni cilt kanseri iken 18inde ise neden yanık ve travma idi. Defektin özellikleri esas alınarak yeniden yapılandırma seçeneği belirlendi. On sekiz hastada tam kat kalınlıkta deri grefti, 19 hastada kompozit greft, 47 hastaya da lokal flep uygulandı. Defektlerin lokalizasyonları, tümörlü hastalarda nüks oranları, yeniden yapılandırma sonrası komplikasyonlar ve yapılan cerrahinin fonksiyonel ve kozmetik sonuçları değerlendirildi.Bulgular: Deri grefti ile yeniden yapılanma uygulanan 7 hastada ve kompozit greft kullanılan 2 hastada tekrarlayan greftleme gerekli olmuştur. Lokal flep ile alt gözkapağı yeniden yapılanma uygulanan hastalardan birinde alt gözkapağı yetmezliği görülmesi nedeni ile kompozit greft kondu. Dört cilt kanserli hastada nüks görülmüştür.Sonuç: Göz kapağı çevresi bölge yeniden yapılandırma estetik ve fonksiyonel açıdan önemli üniteler içermesi ve anatomik olarak bu bölgede mevcut olan kombine dokuların yeniden yapılandırma sürecinde taklidinin zorlu olması nedeniyle, okuloplastik cerrahların sık olarak güçlükler yaşadığı konulardan birisidir. Bu bölgenin yeniden yapılandırmasında, cerrah tarafından mevcut olan yeniden yapılandırma alternatiflerinin iyi bilinmesi ve defektin nedeni, lokalizasyonu, ihtiyaçları, cerrahi sonrası olası problemlerin dikkate alınması ile fonksiyonel ve estetik açıdan optimum sonuçların elde edilebilecektir.
Objective: Between January 1999 and April 2003 the reasons and results of the periorbital reconstruction that was performed in our clinic were investigated retrospectively and advantages and disadvantages of the existing reconstruction alternatives were put forward and subjects that was necessary to be paid attention during reconstruction were determined.Material and Methods: The reconstruction of the periorbital region was performed to 84 patients in fifty- three months. Sixty-two of 84 patients had skin cancer and 18 had periorbital deformity due to the burn and trauma. Reconstruction alternatives were determined according to the defects characteristics. Eighteen patients were reconstructed with full thickness skin grafts whereas 19 had composite grafts and 47 had local flaps. The localization of defects, recurrence rates of tumors, complications and functional and cosmetic results of the reconstruction were evaluated.Results: Re-grafting was required for 7 patients whose defects were reconstructed with skin graft and for 2 patients whose defects were reconstructed with composite graft. One of patients, whom underwent lower lid reconstruction with local flaps, was later reconstructed with composite graft due to the insufficiency of lower lid. Four patients had recurrence of skin cancer. Recurrence of skin cancer was seen in 4 patients.Conclusion: Periorbital reconstruction is one of the subjects that plastic surgeons frequently encounter difficulties, due to the aesthetic and functional important units included in this region and, the various types of tissues present as they are hardly imitated. For reconstruction of this region, reconstruction alternatives must be known very well by the surgeon and etiologies and localizations of defects, requirements for reconstruction and probable postreconstructional problems must be taken into consideration to achieve optimal results from functional and aesthetic aspects.
