Özet
Doğum öncesi, doğum sırası ve sonrası süreçlerde anne ile güvenli bağ kurmanın bebeğin bilişsel, sosyal ve duygusal gelişimi için kritik bir önemi vardır. Bütün yaşamı etkileyen en önemli dönemde, erken doğumla dezavantajlı bir başlangıç yapan annelerin deneyimlerini anlamak, anne-bebek ruh sağlığı açısından önemlidir. Bu çalışma, erken doğum yapmış ve bebeği yoğun bakımda tedavi görmüş annelerin; süreci zihinsel, duygusal ve bedensel açıdan nasıl deneyimlediklerinin ve bu deneyimin bebekleriyle olan ilişkilerini ve annelik deneyimlerini nasıl etkilediğinin anlaşılması amacıyla gerçekleştirilmiştir. Araştırmacı için bilginin sadece zihinden gelen şeyler olmaması ve bedensel duyumsamaların da araştırma için çok önemli bir kaynak oluşturmasından yola çıkılarak, bu araştırmada somatik yaklaşım benimsenmiştir. Araştırmada, İstanbul’da bir vakıf üniversitesi hastanesinin yoğun bakım ünitesinde bebeği tedavi görmüş 10 anne ile derinlemesine görüşmeler yapılmıştır. Görüşmelerde etik hassasiyetlere özen gösterilmiş, görüşmelerin anneleri travmatize etmemesi, tersine anneler için destekleyici olması adına çeşitli önlemler alınmıştır. Annelerin söylemleri, yorumlayıcı fenomenolojik analiz yöntemiyle analiz edilmiştir. Analizler sonucunda; “prenatal dönem, perinatal dönem, postnatal dönem, sağlık personeli ve hasta ilişkisi (anne), prematüre ve yoğun bakım bebeği annesi olmak, yoğun bakım süreci bittiğinde anne-bebek ilişkisi ve hissedilen bedensel duyumsamalar” ana temalarına ve bu temalara ait alt temalara ulaşılmıştır. Erken doğum deneyiminin annelerin duygu, düşünce, algı ve bedensel duyumsamalarını etkilediği; bu etkinin ise prematüre bebekleriyle ilişkilenmelerine ve annelik tutumlarına yansıdığı görülmüştür. Ayrıca, annelerin duygusal olarak zorlandıkları süreç boyunca iç ve dış kaynaklarından faydalanmalarının onlar için oldukça destekleyici olduğu görülmüştür. Bulgular dikkate alınarak, bu kritik dönemde başta anne olmak üzere aile bireylerini destekleyen ve kapsayan pratiklerin yoğun bakım ünitelerine entegre olması önerilmektedir.
Building a secure bond with the mother during the prenatal, perinatal and postnatal processes has a critical importance for the cognitive, social and emotional development of the baby. During the most important period affecting the whole life, understanding the experiences of mothers who made a disadvantageous start with preterm birth is of significance for mother-baby mental health. This study was carried out in order to understand how mothers who gave birth prematurely and whose babies were treated in neonatal intensive care unit experience the process mentally, emotionally and physically and how this experience affects their relationships with their babies and their maternal experiences. A somatic approach was adopted in this research, considering that knowledge is not only something that comes from the mind for the researcher, and that bodily sensations are also a tremendously significant resource for the research. In this study, in-depth interviews were conducted with 10 mothers whose babies were treated in the neonatal intensive care unit of a foundation university hospital in Istanbul. These interviews were conducted adhering to the ethical principles. Measures were taken for the sessions not to be inducing trauma to the mothers, but to be supportive for them. The mothers' discourses were analyzed with thematic and interpretive phenomenological analysis methods. As a result of the analysis; the main themes of "prenatal period, perinatal period, postnatal period, health professionals and patient relationship (mother), being the mother of a premature and neonatal intensive care unit baby, mother-infant relationship after discharge and bodily perceptions felt” and sub-themes belonging to these themes were reached. It was observed that premature birth experience affected mothers' emotions, thoughts, perceptions and bodily sensations; and this effect was reflected in their relationship with their premature babies and their maternal attitudes. In addition, it was understood that it was very supportive for mothers to benefit from their internal and external resources during the emotionally challenging process. Considering the findings, it is recommended that supportive practices involving family members, principally mothers, should be integrated into neonatal intensive care units during this critical period.
