Özet
Bu araştırma, bypass cerrahisi geçiren hastalarda toraks tüpü çıkarılması işlemine bağlı gelişen ağrıda soğuk uygulamanın etkisini belirlemek amacıyla, Ağustos 2021- Kasım 2021 tarihleri arasında Özel Acıbadem Altunizade Hastanesi Kardiyovasküler Cerrahi Anabilim Dalı’nda ameliyat olan 56 hasta ile gerçekleştirildi. Bu hastaların 28’i müdahale grubunu, 28’i kontrol grubunu oluşturdu. Araştırma verileri için, ‘‘Hasta Tanılama Formu’’, ‘‘McGill Ağrı Ölçeği Kısa Formu’’,‘‘Cilt Sıcaklığı Değerlendirme Formu’’ ve ‘‘Yaşamsal Bulgular Takip Formu’’ ile toplandı. Toraks tüpü çıkarılma işleminden 20 dk. önce ve işlemden sonra 20 dakika soğuk uygulama yapılarak, ağrı şiddetine etkisi değerlendirildi. Veriler SPSS 25 program ile analiz edildi. Örneklem sayısı 30’dan küçük olduğundan dolayı Non-Parametrik testler uygulandı. Uygulanan analizler %95 güvenilirlik düzeyinde yapıldı. Ki-kare analizi ile müdahale ve kontrol grubundaki dağılımlara, Mann Whitney-U testi ile iki grubun karşılaştırılmasına, Friedman Testi ile bağımlı ölçümlere bakıldı. Her iki grubun çoğunluğunun orta yaş, şişman, sigara içen, erkek hastalardan oluştuğu görüldü. İşlemden 20 dakika öncesi, işlem sırası ve işlemden 20 dakika sonra grupların çoğunlukla ağrıyı tüp bölgesinde hissettikleri, gruplar arasında ağrının niteliği açısından farklılık olduğu görüldü (p<0.05). Grupların işlemden 20 dakika önce ağrı şiddeti, cilt ve vücut sıcaklıklarının aynı olduğu, müdahale grubunda ağrının şiddeti, cilt ve vücut sıcaklıklarında işlem sırası ve işlemden 20 dakika sonrasında azaldığı belirlendi. Gruplar arasında sistolik kan basıncı, diyastolik kan basıncı, nabız, solunum değerlerinde anlamlı fark olmadığı görüldü (p>0.05). Kontrol grubunda sistolik, diyastolik kan basıncı ve nabız hızının işlem sırasında arttığı görüldü. Soğuk uygulama yapılan hastaların SpO2 düzeyinin daha yüksek olduğu görüldü. Araştırma bulgularına bakıldığında soğuk uygulamanın ağrının yönetiminde etkili olduğu görüldü. Soğuk uygulamanın soğuğa karşı hassasiyeti veya alerjisi olmayan hastalarda uygulanması önerilir.
This study was carried out with 56 patients who underwent surgery in the Cardiovascular Surgery Department of Private Acıbadem Altunizade Hospital between August 2021 and November 2021, in order to determine the effect of cold application on pain caused by thorax tube removal in patients undergoing bypass surgery. Of these patients, 28 were the intervention group and 28 were the control group. For research data, "Patient Diagnosis Form", "McGill Pain Scale Short Form", "Skin Temperature Evaluation Form" and "Vital Signs Follow-up Form" were collected by face-to-face interview method. Cold application was performed 20 minutes before the removal of the thorax tube and 20 minutes after the procedure and its effect on pain intensity was evaluated. Data were analyzed with SPSS 25 program. Non-Parametric tests were applied because the number of patients was less than 30. The analyses were performed at 95% confidence level. Distributions in the intervention and control groups were examined with chi-square analysis, comparison of the two groups with the Mann-Whitney-U test, and the dependent variables were measured with the Friedman test. It was observed that the majority of both groups were middle-aged, overweight, smoking, male patients. They felt the pain mostly in the tube region 20 minutes before, during and 20 minutes after the procedure, and there was a difference in the quality of pain between the groups (p<0.05). It was determined that the pain intensity, skin and body temperatures of the groups were the same 20 minutes before the procedure, and the severity of pain, skin and body temperatures in the intervention group decreased during the procedure and 20 minutes after the procedure. There was no significant difference in systolic blood pressure, diastolic blood pressure, pulse and respiratory values between the groups (p>0.05). It was observed that systolic, diastolic blood pressure and heart rate increased during the procedure in the control group. It was monitored that the SpO2 level of the patients who had cold application was higher. Considering the research findings, it is seen that cold application reduces pain and it is recommended for patients who are not sensitive or allergic to cold.
