| dc.description.abstract | Konu: Travma bilgili yaklaşım, travmanın etkisini anlamaya ve bu etkiye yanıt vermeye dayanan hem hizmet sağlayıcılar hem de hayatta kalanlar için fiziksel, psikolojik ve duygusal güvenliği vurgulayan ve hayatta kalanların kontrol ve güçlenme duygusunu yeniden inşa etmeleri için fırsatlar yaratan güçlü yönlere dayalı bir hizmet sunum şeklidir. Travma bilgili yaklaşımın ruhsal travmanın yaygınlığını anlama, stres tepkilerine doğru yaklaşım, yeniden travmatizasyonu önleme ve çalışanların iyilik halini koruma ve sürdürme şeklinde dört bileşeni bulunmaktadır. Araştırmalar hastane öncesi acil bakımda çalışanların travma sonrası stres bozukluğu geliştirme açısından risk altında olduklarını ortaya koymaktadır. Bununla birlikte hastane öncesi acil bakım çalışanlarının iyilik halinin yükselmesinin hasta/yaralının yeniden travmatize olma riskinin azalttığı görülmektedir.Amaç ve Kapsam: Bu çalışmada hastane öncesi acil sağlık hizmetlerine entegre etmek için geliştirilen ‘travma bilgili acil sağlık hizmeti sunumu’ eğitiminin kapsamını ve uygulama sonuçlarını değerlendirmek amaçlanmıştır. Eğitim, davranış teorilerine dayalı ruhsal travmanın epidemiyolojisi ve etkileri, travmatik yaşantılarda ortaya çıkan tepkiler, travmatik stres tepkilerine yaklaşım ve müdahale araçları, psikiyatrik acillere yaklaşım, afet öncesi hazırlık, çalışan sağlığını koruma ve geliştirmeyi içeren 40 saatlik bir programdır. Yöntem: Araştırma grubunu İstanbul İl Sağlık Müdürlüğü İl Ambulans Servisi Başhekimliği bünyesinde çalışan 19 paramedik oluşturmuştur. Araştırma yarı deneysel desende tasarlanmıştır. Eğitim Temmuz 2024'de verilmiş, etki değerlendirmesi 3 ay sonra yapılmıştır. Bulgular: Eğitime katılan 19 paramediğin %37,5 i kadın, %62,5’i erkektir. Katılımcıların yaşları 2246 arasında olup, ortanca yaş 26.5’tir. Katılımcılar formda bulunan tüm maddelerden 4-5 arasında puan almış olup, ortanca puan 5'dir. Katılımcılar bu eğitim sayesinde; travmatik olaylardan sonra hasta, yakınları ve ekip arkadaşlarında ortaya çıkan zihinsel, bedensel, duygusal, davranışsal ve sosyal stres tepkilerini daha iyi anladıklarını, daha etkili iletişim kurduklarını, eğitimin zorlu çalışma koşullarında sabırlı olmalarına yardımcı olduğunu, iş performansını arttığını, aile ilişkilerine olumlu katkısının olduğunu, tükenmişliği önlemede yardımcı olduğunu bildirmişlerdir. Sonuç: Bulgulara dayalı olarak travma bilgili yaklaşımın hastane öncesi acil bakım süreçlerinde etkili olduğu görülmüştür. Bu kapsamda travma bilgili yaklaşımın hastane öncesi acil bakım süreçlerinde çalışan bireylere verilmesi önemlidir. Bu eğitimin Sağlık Müdürlükleri Çalışan Sağlığı Birimleri yanı sıra, Toplum Sağlığı Merkezlerinde görev yapan sosyal hizmet uzmanlarına verilebilir. Bu sayede güçlendirme temelli kurumsal mekanizmaların oluşturulmasına katkı sunulacağı düşünülmektedir. | |
| dc.description.abstract | Subject: The trauma-informed approach is a strengths-based service delivery approach that emphasizes physical, psychological, and emotional safety for both providers and survivors, based on understanding and responding to the impact of trauma, and creates opportunities for survivors to rebuild their sense of control and empowerment. The trauma-informed approach has four components: understanding the prevalence of psychological trauma, addressing stress responses appropriately, preventing retraumatization, and protecting and maintaining the well-being of employees. Research has shown that prehospital emergency care workers are at risk for developing posttraumatic stress disorder. Aim and Scope: This study aimed to evaluate the scope and implementation results of the 'trauma-informed emergency health service delivery’ training developed to integrate into pre-hospital emergency health services. The training is a 40-hour program that includes the epidemiology and effects of psychological trauma based on behavioral theories, reactions that occur in traumatic experiences, approaches and intervention tools to traumatic stress reactions, approaches to psychiatric emergencies, pre-disaster preparation, and protection and development of employee health.Methods: The research group consisted of 19 paramedics working in the Istanbul Provincial Health Directorate Provincial Ambulance Service Chief Physician. The research was designed in a quasi-experimental design. The training was given in July 2024, and the impact assessment was made 3 months later. Data were collected online via Google Forms, and the Trauma-Informed Approach to Emergency Cases Form, which was developed by the researchers and scored on a 1-5 Likert type, was used in data collection. In addition, the opinions of the paramedics who participated in the study regarding the content of the training were obtained after the training. Results: Of the 19 paramedics who participated in the training, 37.5% were female and 62.5% were male. Participants were between 22 and 46 years old, with a median age of 26.5. Participants scored between 4 and 5 on all items in the form, with a median score of 5. They reported that they better understood the mental, physical, emotional, behavioral, and social stress reactions that occur in patients, their relatives, and teammates after traumatic events, that they communicated more effectively, that it helped them be patient in difficult working conditions, that it increased their job performance, that it had a positive impact on family relationships, and that it helped prevent burnout. Conclusion: Based on the findings, it was observed that the trauma-informed approach was effective in pre-hospital emergency care processes. In this context, it is important to provide the trauma-informed approach to individuals working in pre-hospital emergency care processes. This training can be given to social workers working in Health Directorates' Employee Health Units as well as Community Health Centers. It is thought that this will contribute to the establishment of institutional mechanisms based on empowerment. | |
